Dartmouth Health Cheshire Medical Center, located in beautiful Keene, NH, is seeking a compassionate and motivated Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our multidisciplinary team in our outpatient diabetes clinic. This is an exciting opportunity to care for patients living with diabetes in a supportive, patient-centered environment focused on education, prevention, and long-term disease management.
Key Responsibilities:
- Provide comprehensive evaluation, diagnosis, and management of patients with diabetes in an outpatient setting
- Collaborate closely with endocrinologists, diabetes educators, dietitians, and nursing staff to deliver holistic patient care
- Educate patients and families on diabetes management, lifestyle modifications, and medication use
- Order and interpret labs, adjust medications, and monitor treatment outcomes
- Participate in quality improvement initiatives and contribute to program growth
Qualifications:
- Nurse Practitioner (NP) or Physician Assistant (PA) in New Hampshire
- Certification in Adult, Family, or Acute Care (NPs) or NCCPA-certified (PAs)
- Prior experience in endocrinology, diabetes care, primary care, or related specialty
- Strong clinical judgment, communication, and patient education skills
