Dartmouth Hitchcock Medical Center is seeking an experienced Nurse Practitioner or Physician Assistant to join our interdisciplinary Academic Geriatric team to provide support at our assisted living and nursing home facilities in addition to clinic work. 

In this role you will provide primary healthcare to individuals and families and assess and manage the healthcare needs and problems within their specialty area, emphasizing health promotion and disease prevention as well as the diagnosis and treatment of acute and chronic illness. You will function as part of the provider team to provide care to the department’s panel of patients.

Your involvement in working within an interdisciplinary team is an essential aspect of this position but also the ability for you to work autonomously. You will see patients in a variety of settings including clinics, assisted living, skilled nursing units, and sometimes within their homes.

Prior advanced practice nurse experience is required which should include primary care experience. Experience in geriatrics/palliative care is preferred but candidates with other comparable experience and committed interest in geriatrics and palliative care will be considered. This position does require on-call commitments shared amongst the other team providers.
